Once your appointment is scheduled on line - you will be able to download a word or PDF file called "Mini Intake Forms" + "Mini Assessment Forms". The forms are easy to fill out and are to be emailed or mailed back. 12 of the body's systems are evaluated, as well as the organs within those systems. For Example: The Digestive System, and within that system: Liver, Pancreas, Gall Bladder, Stomach, Blood Sugar Balance. This package also has a 30 minute follow-up & Takes in your medical history & medications. A mini Lifestyle & Dietary Assessment is included. Very Informative Process. Good for Prevention as well as Revealing Red Flags and things that may require more testing. For Example via tests at the Centre or Blood Tests via your Doctor. This can also help to answer questions or valid concerns you many be experiencing. Plan set in motion for your health goals.
